The Kiwanis Club of Waldorf Maryland Foundation, Inc. will act as a “safety net” for children and families of children that have catastrophic medical situations. When all other local, state and federal assistance is exhausted, the Foundation may proved support. The Foundation officers and directors will evaluate, investigate and react to the needs of children and their families to alleviate the financial, physical and emotional pain associated with childhood illness or injury.The Foundation does not discriminate in regard to race, religion, color, gender, nationality, sexual OIrientation, or physical challenge.
CRITERIA
Families applying for assistance
from the Kiwanis Club of Waldorf, Maryland Foundation, Inc. hereafter known as
“the Foundation” must meet the following criteria:
1.
Be
able to provide proof of residency in Charles County, Maryland.
2.
Show
proof of having the legal responsibility (as Parent(s) or Legal Guardian(s))
for an ill or injured child between the ages of birth and 18 years of age.
3.
Provide
documentation for the need of financial assistance because of the fact that
they are caring for a child experiencing illness or injury, life threatening or
not.
4.
Be
willing to provide information to the Foundation to validate the request for
funds including the name of the child’s physician(s);
5.
Be
willing to provide information regarding the expenses to be paid to the service
provider from the Foundation’s funds;
6.
List
all other sources of funds which could be used to help pay the expenses of the
ill or injured child. Example: Income, Insurance, Government Agencies,
Non-profit Agencies, etc.
